Go to Wufoo.com and sign up, after that you can create a new form.

Add it to your website or blog and the submissions will be displayed in your Wufoo.com form manager.
WordPress plugin
Wufoo offers many different ways to add the guest post form to your blog or website. You can link to the form, which makes it open in a new window, hosted on wufoo.com. Or, you can embed the code in a post, on a page or in a widget. By either using javascript, an iframe or the plugin available to many different CMS’s, like WordPress, Joomla and Drupal to name a few.
WordPress Note: When you’re done with the form and are in the Form Code Manager, you simply click on Embed Form Code, in the menu. Select & copy the WordPress Shortcode at the bottom and paste it in a newly created page, hit save and you’re done.
